WMRHA AHCM 2022 Symposium We are so excited to celebrate our five-year Accountable Health Communities Model Journey (2017-2022) with each of you. It is bittersweet that AHCM is ends this April, but the excellent work continues with the Community Resource Network (CRN). Not only can partners screen for social needs, but also shows when referrals are received, in process, or resolved along with the ability for a care team among health care providers, government, and community partners. More partners are joining, so if you want to learn more, contact me at [email protected] for more information. On April 18th, we will celebrate the accomplishments of all of us in the last five years. Considering lifted COVID-19 restrictions, we will have an in person West Mountain AHCM local symposium to recognize all of you for your hard work, dedication, and your resolve in this project despite so many unforeseen challenges. We would like to have you join us in person. We do ask for you to register as we have a limit to the number we can have in-person at the event. An event invite will be sent your way. Please register no later than April 4th. For those unable to join in person you are invited to attend the entire event or part via Zoom from 10:30-1:30 (there will be a couple breaks), and the AHCM recognition will happen from 12-1p.
