We are conducting a study of how businesses use mechanical assistance in lifting or carrying heavy items or objects at the workplace or on site.   Please complete this entire survey honestly and you will qualify for a chance (1 out of every 5 respondents) to win a $50 Visa gift card (only one gift card will be awarded per person, and who has given valid responses).  In addition, there will be one drawing for $250. You will also have a chance to have a 10 minute interview, where you will be paid $50 immediately via PayPal or Venmo. Only one per household and survey must be honestly and completely filled out.
