I am conducting a study on individuals living with a heart condition that includes self assessment of living with a heart condition (time of diagnosis, number of medications, number of surgeries), personal characteristics, mental and physical health. The study takes 30 minutes to complete and can be done through an online survey. To be eligible to participate in this study you must be 18 years old or older, be able to complete the questionnaires yourself, and have been diagnosed with a heart condition.
