Press release from the University of Houston-Downtown: March 10, 2022 The University of Houston-Downtown and H-E-B are hosting a COVID-19 Vaccination Clinic for the UHD Community and the public on Tuesday, March 22 from 11 a.m. - 1 p.m in the UHD Welcome Center Lobby. The following vaccines will be available:PfizerModernaJohnson & JohnsonShingles (recommended for ages 50+) Important Reminders: Everyone must complete a paper copy form prior to them coming for their appointment. English Form | Spanish Form When registering online, in order for us to identify which vaccine a person wants, we ask that they document in parentheses after their first or last name. (Ex. John (Pfizer booster)) Everyone must bring their pharmacy and medical insurance cards like the other clinics. Bring an identification card that matches your appointment name and wear a mask during your immunization. Be sure to bring your COVID-19 vaccine record for an additional dose or booster appointment. If you received your first dose somewhere other than H-E-B, please be sure to indicate the manufacturer and date of your first dose when completing your registration. If your first dose was with H-E-B, you will be auto-scheduled for your second dose. Parking is free for 0 to 30 minutes in our Visitor Parking | 201 Girard Street Houston, TX 77002. Must show appointment confirmation upon exit. *After 31 minutes you will be charged a flat rate of $5. Hand sanitizer stations and masks are available upon request. For your convenience, the Curative testing kiosk outside the Jesse H. Jones Student Life Center is still open, seven days a week from 8 a.m. to 6 p.m. and free of charge.
