Looking for a position where you can make a difference? CTF Illinois is an essential employer seeking a QIDP to support individuals with developmental disabilities at our Crestwood location. Overview: General work schedule will be Monday – Friday. This will vary in order to meet the needs of individuals receiving services.Design and provide quality services and programming to enhance each individual's skill development, health, freedom, and personal dignity.Support self-advocacy and self-determination of individuals by giving them the opportunity to make choices and encouraging empowerment whenever possible within the guidelines of the Individual Service Plan and Behavior Support Plan (if applicable).Assure that each individual served receives an Individual Service Plan (ISP) annually, or more often as needed, by scheduling the necessary Interdisciplinary Team (IDT) members and chairing the meeting.Implement each individual ISP by developing a written plan, goals, and objectives based upon the assessed strengths and needs of each individual, and by defining systems to monitor progress.Schedule and attend psychiatric visits for individuals receiving psychotropic medications.Ensure all individuals have medicaid cards, Ensure activity and advisory council meetings are conducted as required.Complete daily documentation related to implementation and monitoring of the ISP for individuals receiving services.Coordinate with nursing and other professionals to ensure health of each individual is maintained.When individuals are present, coordinate services in conjunction with the site supervisors.Ensure each individual's rights are maintained by adhering to the policies and procedures set forth by the company and regulatory agencies.Maintain each individual's personal history book by completing compulsory forms as prescribed by policy, obtaining reports from outside providers, filing materials in a timely manner and assisting with the completion of regular quality assurance reviews.Assure that adaptive equipment (including wheelchairs) is maintained in good condition by monitoring the use of such items.
