Salam everyone! We are so excited to host another Mommy & Me Event! All mothers are welcome to join us with their kids (ages 0-5), to enjoy circle time, snacks, crafts, and more! This will take place on Sunday, March 20th at 2:15PM - 4:15PM in the IAGD Banquet Hall. 1 child = $10 2 children = $15 Please see event terms and conditions below: As a prerequisite condition of participating in any Muslim Youth of Greater Detroit (henceforth “MYGD”) programs, I hereby waive, release, and discharge any and all claims for damages, personal injury, and/or property damages which may henceforth occur to me and/or my participating dependent(s) as a direct and/or indirect result of participation in said programs. This release is intended to, in advance, discharge MYGD and its parent organization (IAGD), as well as their respective officials, employees, volunteers, and agents, from liability in the event of such damage, be it the result of negligence on the part of the aforementioned or otherwise. It is understood that some recreational activities involve an element of risk or danger of accidents, and I hereby, in the full knowledge of these risks, assume them. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding upon, in addition to myself, my heirs and assignees. To the best of my knowledge, I am in good physical condition and health, and I have no knowledge of any physical condition, injury, or illness whatsoever which would place me at risk should I participate in the program for which I sign this waiver form. I consent to the use of quotes and the taking of photographs, movies, and/or video tapes of myself and/or my dependent(s) by MYGD. I also grant to MYGD the right to edit, use, and/or distribute said products commercially and/or non-commercially, including use in print, on the internet, and all other forms of media. Should a minor or minors accompany me at any time, I agree that I shall supervise them in accordance with MYGD’s policy, and shall be responsible for their conduct at all times while they are on the premises. I agree to abide by any additional regulations dictated by MYGD at the event, and accept that MYGD reserves the rights to deny me and/or my dependents entry thereto and to evict me and/or my dependents therefrom at any time, for or without cause. In no event shall MYGD be liable for any claim, damages, or other liability, whether in an action of contract, tort, or otherwise, arising from, out of, or in connection with participation in its programs.
