WAUKESHA, WI — The deadline for families of victims, people injured, and others impacted by the Nov.21 Waukesha Christmas parade is Monday. The Waukesha County Community Foundation, United Way of Greater Milwaukee and the Waukesha Education Foundation established the United for Waukesha Community fund to raise money for those impacted after a man drove through marchers and spectators, killing six people and injuring more than 60. Those eligible have to submit either the deceased victim claim form or the uniform claim form selecting one of the categories listed below: Claimants for people who died as a result of the parade will receive the highest category of payment.Individuals injured who required overnight hospitalization.People treated for injuries at Waukesha and Milwaukee County area hospitals.People present during the parade such as first responders and medical professionals treating those injured at the time of the parade.Nonprofit agencies serving people impacted by the parade.Nonprofit agencies providing mental health services for people impacted by the parade. The form asks for basic information such as name, address, and phone number. It also asks for supporting documents such as birth, death certificates and a note on hospital letterhead. The uniform claim form must be submitted by Monday.
