Along with your photo ID, accepted forms of proof will be: - your vaccination card - a clear & legible photo of your vaccination card - a clear & legible photocopy of your vaccination card - a digital record or app such as Vax Verify / MyChart The state of Illinois also remains under an indoor mask mandate.
