Need Certified PRI/Screen Person

We live in NY and are trying to transfer my mom from a nursing home in Farmington to a nursing home in central NY. The issue is that NY requires a specific form be filled out by a certified PRI/Screen person. So we are looking for someone in the Farmington area who is certified and can complete this form for my mom at her nursing home. Please contact me if this is something you are certified to complete.
