Someone to bust up concrete and haul it away, also grading

I have a thick concrete slab, about 3 feet x 3 feet and 12 inches thick. This needs to be busted up so it can be taken away in pieces. The slab is too thick for a sledgehammer to do much with. You would probably need either 3 guys or one jackhammer to bust it up, or maybe a hammer drill to drill holes in it and then hit it with a sledgehammer. Once that's busted up I have some other stuff needing to be hauled away as well. The one photo shows both piles which are both the ones by the street. The closer up view shows the one with the cement slab. Also need someone else who can do landscape grading. Property is already relatively smooth but it needs scraped of all the roots a bobcat recently dug up and smoothed out. 2 photos of yard shown.
