Press release from City School District of Albany: February 4, 2022 The City School District of Albany is parterning with the Albany County Health Department to hold COVID-19 vaccination clinics at two district schools during February -- one this Monday and one Feb. 17. Monday's clinic will be held at Edmund J. O'Neal School of Excellence, 50 Lark St., from 4:30-7 p.m. Registration is required for the Feb. 7 clinic; sign up here. The Feb. 17 clinic will be held at Philip J. Schuyler Achievement Academy, 676 Clinton Avenue, from 4:30-7 p.m.. Registration is suggested for the Feb. 17 clinic; sign up here. First and second doses For people age 5 and older (pediatric dose for age 5-11; adult dose for 12 and older) Children and teens age 5-15 must be accompanied by a parent or guardian Teens age 16-17 must bring a consent form signed by a parent or guardian.  The Pfizer COVID-19 vaccine is a two-part vaccine. A second dose will be scheduled at the time of the first dose Third dose  For eligible people who have received both the first and second dose of Pfizer vaccine. Please bring vaccination card or Excelsior Pass Plus as proof of vaccination Booster shots For people age 12 and older who received their first and second Pfizer shots at least five months ago. Please bring vaccination card or Excelsior Pass Plus as proof of vaccination First and second doses For people age 18 and older The Moderna COVID-19 vaccine is a two-part vaccine. A second dose will be scheduled at the time of the first dose. Third doses For eligible people who have received both the first and second dose of Moderna vaccine. Please bring vaccination card or Excelsior Pass Plus as proof of vaccination Booster shots   For people age 18 and older who received their first and second Moderna shots at least five months ago. Please bring vaccination card or Excelsior Pass Plus as proof of vaccination Arrive wearing a mask and practice social distancing regardless of vaccination status No insurance, social security number or medical paperwork needed No fee or cost for the vaccine – it is FREE If you have any additional questions or concerns, please contact the Albany County Health Department COVID-19 Call Center at (518) 447-4659.  This press release was produced by City School District of Albany.
