Free sports physicals will be provided to any high school student athlete participating in sports within Kansas City Public Schools.
This registration link is to be utilized by coaches, athletic directors and athletic trainers to register a student athlete for a sports physical. A coach, athletic director or athletic trainer must verify the student athlete's parent has completed pages 2-3 of the MSHSAA Pre-Participation Physical form and the medical authorization form prior to registration. These forms can be completed on paper or on PRIVIT. Paper copies of both forms will need to be brought with the student athlete to his or her physical exam. Any student athlete without the proper paperwork will be turned away.
