Description

SHIFT is a free interactive training for families and youth with disabilities ages 14-21 who are navigating the transition to adulthood. All local and state COVID protocols will be followed. Join us as we explore person-centered planning and learn about the transition process, Vocational Rehabilitation services, Independent Living Centers, Medicaid waivers and so much more. Families and youth will leave with a plan outlining next steps for their transition from high school to adult life. You will leave with tips, resources, and information to help navigate: The Road to the Good Life. Explore ways to use person-centered planning as part of the Individualized Education Program (IEP). Gain a better understanding of how to use the transition to adulthood process to prepare students for life after high school.The Road to Careers and Employment. Learn how Vocational Rehabilitation and Pre-Employment Transition Services can assist youth in gaining and maintaining employment.The Road to Independence and Interdependence. Gain information for accessing supports and services that will help the youth in their communities. Families Together is Kansas’ federally designated Parent Training & Information Center (PTI) and Family-to-Family Health Information Center (F2F). We provide support for families and their children who have disabilities, and/or special health care needs.
