FREE COVID Testing | Rapid & PCR Testing There is a pop up covid clinic at City Hall/Recess in the West Loop for anyone looking for Rapid and PCR test. They will be open all month long from 10 am - 6 pm daily offering self administered tests Tuesday - Sunday. Time: 10 am - 6 pm Walk in appointments available. Registration is preferred! In order to speed up the process onsite, can you please fill out the patient form prior to coming to your appointment. Link to form: https://loop.labsvc.net/patientportal/index.html?artevent Click on Order Test • Fill out name & Address • check box for "I don't have insurance" • You do not need to upload your driver's license • Enter Email | Password > This will be your login to check your PCR Test Results • Click on Text or Email verification # • Verify Account • Once account is verified you will be asked a series of questions • When selecting the test options choose: SARS-COV-2 BY RT-PCR $0.00 • Answer the questions accordingly for the next 7 pages • Once you get to payment options click on the only option " I'd like to use my insurance or I am uninsured" • It will direct you to a page to choose a patient, please select yourself or the person you are registering for. • Scroll down to the bottom of the form and answer the question if you are a citizen or a green card holder. • Electronic sign your name • Pick no appointment on the last page when they ask for time • Click the purple button on the top that says "Click here if you do not need an appointment" • Please show this QR code at check in at the testing site. ****Please wear a mask and there are no bathrooms on-site so please come prepared.
