Role Type: Community Outreach Specialist (Contract) Pay Range: $20.00 - $25.00/hour Looking for a fast-paced and rewarding career in healthcare? We are looking for dedicated community leaders to join our team as a Community Outreach Specialist. You would join our Home Visits team! The role will start as a PT contract but there is potential for this to evolve into a FT, permanent position. General Description: We are looking for enthusiastic Community Outreach leaders to join our Home Visits Team. The role offers: -part-time contract with a flexible schedule -opportunity for the role to evolve into a FT, permanent position -great entry-level opportunity for those looking to get into the healthcare field -community outreach training -competitive pay As a member of our Community Outreach Team, you will be responsible for visiting health insurance recipients to ensure that they are not losing coverage or to obtain medical records signatures. We will provide you with all the training you need to succeed! Responsibilities: -Conduct outreach to health insurance recipients to ensure member retention, answer questions, or obtain signed medical records -Collect & Communicate additional member contact information, as applicable -Clearly report back the day's activities by logging your data appropriately -Participate in required 1:1 meetings and team meetings -Performs other field duties as may be assigned by management Qualifications: -Associate's degree (preferred) -accustomed to working with vulnerable populations on Chicago's southside and surroundings areas -1-2 yrs community outreach experience -Can travel throughout the City of Chicago and surrounding suburbs -Currently live within the City of Chicago or comfortable working in the city and surrounding suburbs -Have reliable transportation (own vehicle) -Community organizer or restorative justice background is helpful -Ability to work independently and manage time effectively -Access to a TABLET OR COMPUTER WITH DATA/INTERNET*
