The community prevention organization's brief, voluntary, seven-question survey, is estimated to take under five minutes from start to finish. The survey, which can be accessed by the QR code below, helps the coalition to "gather information about adult attitudes toward youth drug and alcohol use:" It is also linked directly on the Madison Chatham Coalition homepage here; or found at www.madisonchathamcoalition.org.
