Join us to make a difference!Job Responsibility:-Does client paperwork from Admission to Discharge to include Outcome and Assessment Information Set (OASIS) to Discharge OASIS.-Provides hands-on care, case management, medication updates, evaluation & reevaluation of the care plan and teaching of the patient, family, caregiver in accordance with physician orders as required by CHAP, our policies and other regulations.-Reports patient's progress and condition to patient's physician and/or Clinical Manager on a continuous basis.-Coordinate care with interdisciplinary staff members to include Physical, Occupational and Speech Therapists; Home Health Aides.-Supervises LPN, HHA, with written updated care plans as needed.-Maintains Agency property, supplies and equipment in a manner that demonstrates ownership and accountability.-Demonstrates infection control per OSHA guidelines and Agency policies.-Ensures compliance with the Medicare Conditions of Participation and any other regulatory requirements.QUALIFICATIONS:-Registered Nurse license in the state of MA-Must possess a valid driver’s license with auto insurance-Minimum of 1 year of med/surg experience-Must have current CPR certificate-Ability to work independently and manage time effectively-Strong organizational and communication skills-Must have 1 to 2 years home care experience with OASIS experience-Good computer skills, prior experience with electronic medical records(EMR)-Remains current for all other Agency mandatories-Maintains professional appearance, treats others with care and respect, and demonstrates excellent customer serviceEden Home Health Care, Inc. is an Equal Opportunity Employer Principals only.
