AHRC Nassau will be offering first dose vaccinations and boosters to employees interested in being vaccinated against COVID-19. If you are interested in receiving your vaccination on January 27th, 2022 at the Plainview ICF, please fill out the required information to schedule your appointment. This clinic will begin at 11:30AM promptly and conclude at approximately 2PM. If we exceed the number of appointments allotted for that time, the vendor will be happy to extend. You will have until close of business on Thursday, January 20th, 2022 to submit the required information and schedule your appointment accordingly. You will need to bring your current vaccination card and health insurance card with you at the time of your appointment. We are asking that you fill out the consent form attached to the email sent out by the agency and bring that with you as well so the event runs in a timely manner. If you are not able to do so, please arrive earlier than your appointment to give you time to complete it beforehand.
