Now Hiring - LPN Case Manager LPN wanted for low-key, low-stress, not on your feet all day job Monday – Friday @ 8 am-4 pm flexible office full-time hours w/ full benefits. No mandatory Overtime or work on holidays. Salary wage negotiable; Office located in Grand Junction, CO. Stable clients & stable work supporting adults with Down Syndrome, Autism, CP, Seizures, etc. & their care providers. Enjoy working with a fun, professional team. Take medically fragile people bowling or on other day program outings. A good sense of humor w/ a winning attitude is a must. This unique LPN position is part administrative work and part practical nursing care: • ADLs care, instruction & advice • G-tube feedings & observations • May need to lead QMAP training (currently using an online service) • Maintain medication administration records • Detailed record keeping of care with strong organizational skills • Basic medical care; recording vitals, wound care • Understanding of mental health/psychiatric care • Phone calls/text/email for nursing concerns may occur evenings or weekends • Some travel to Thornton, CO for training - expenses paid Must be 21 years of age, clean driving record & background check, licensed & insured in CO is a must. $100 bonus for COVID-19 vaccination.
