The Louisville Underground Mandatory Vaccination Policy To keep our patrons, employees, performers, crew and guests safe, beginning Wednesday September 1 , 2021 (until further notice) The Louisville Underground will require anyone entering the facility to be fully vaccinated. No one under the age of 21 is allowed in The Louisville Underground so no vaccination exemptions for age are required. Doors will monitored by staff and no one will be allowed in until status has been confirmed. Wrist bands will be issued upon verification. Sign and ropes will be placed at both entrances. We only accept the following as proof of vaccination. COVID-19 Vaccination Record Card Issued by: Department of Health and Human Services & Centers for Disease Control & Prevention WHO Yellow Card It must include: Name of the person vaccinated § Type of vaccine Date of last dose administered Documentation of COVID-19 vaccination from a health care provider myColorado Digital record Photocopies and digital images are acceptable The Louisville Underground strongly requests that patrons do not attend if they are not feeling well or are showing any COVID like symptoms. By entering the Louisville Underground, guests voluntarily assume all risk associated with COVID-19 exposure. Some of the performers have specific requests for the safety of their crew and fans. We will continue to work with them to ensure appropriate safety measures are in place. Please remember to routinely check our website and social media outlets as individual event requirements may be more stringent.
