Create 26 2-foot x 2-foot concrete forms and pour concrete

hello, I need a concrete expert to create a lot of two foot by two foot forms (a total of 26 forms) in two areas of my front yard. I have provided a drawing, and two pictures of the front of my house. The drawing shows both areas, with measurement. The forms need to be exactly the size of the 2 foot x 2 foot tile shown in the 4th picture. Total concrete is 104 square feet, about 4-5 inches thick. Each forms must have a proper compacted base and rebar. No quickcrete allowed, pour concrete using concrete truck only.
