Only ONE form submission per household, please. Duplicate form submissions with the same IP address WILL BE DELETED.   There are two tests in each test kit.  A maximum of two test kits (four tests total) will be provided per household. Test kits will be provided to Framingham residents ONLY. You will be REQUIRED to show proof of residency. Residents will be able to choose from one of two collection days. Face coverings are REQUIRED when collecting test kits.  If inclement weather occurs, the distribution will be rescheduled. Additional Info...
