There are two (2) Rapid Antigen tests in every box, there will be a limit of 3 boxes (6 tests) per family to make sure every resident of Waltham that needs a test, receives a test. Supplies are limited & the Health Department will be doing their best to prioritize. This form will close when supplies are accounted for.
Tests will be dropped off to your home by a Waltham Health Department employee on (or closest to) the scheduled date and time you choose. Hours for drop off will be between 9:30AM - 3:30PM, Tuesday, Wednesday, or Thursday ONLY.
