Help to level the floor in a 12x18 shed office

Hi, I have a finished office that was a conversion from a shed at my home. The floor slopes pretty bad and I was looking for some help to even it out. The floor is finished -- it's luxury vinyl plank on top of plywood (drycore) panels on top of a concrete pad and slopes a total of about 5 inches from one side of the office to the other. My plan was to pull up the vinyl planks and trim, screw down some long-shimmed two by fours, cover everything with another layer of plywood, and put back the vinyl and trim. The pad itself slopes so there's not much I can do about that. I'll supply all the materials, I just need a hand for a day. Thanks
