Press release from City School District of Albany: December 12, 2021 The Albany County Health Department will administer free COVID-19 vaccines for all eligible individuals age 5 and older at Stephen and Harriet Myers Middle School on Tuesday, Dec. 14, from 4:30-7 p.m.  Registration is required; no walk-ins will be accepted. To register for a COVID-19 vaccination (first, second or third dose, OR a booster shot), please go to this link. For general information about the COVID-19 vaccine, please visit the CDC Fact Page. General clinic reminders: Arrive wearing a mask and practice social distancing regardless of vaccination status All persons aged 5+ years old are eligible to receive the vaccine All persons 15 and under will be required to attend the clinic with a parent or guardian All persons 16 and 17 arriving without a parent or guardian will be required to arrive with a signed NYS COVID-19 immunization Screening and Consent Form No insurance, social security number or medical paperwork needed No fee or cost for the vaccine – it is FREE COVID-19 vaccine formulas and doses offered: If you received your first dose of the Pfizer vaccine at the Myers clinic on Nov. 24, then you are already scheduled to receive your second dose at Myers on Dec. 14 for the same time as your Nov. 24 appointment. An email and/or text reminder will be sent on the evening of Monday, Dec. 13. You do not need to re-register.
