Christmas Gift GiveAway - ALL FREE - Toys, Groceries, Bill Pays, Gift Cards, & More! Special appearance from Santa Claus! The RIver Mount Washington presents: Christmas Gift GiveAway When: Saturday, December 18th, 2021 Time: 11am - 3pm (Eastern Standard Time) Where: 1499 Wales Run Road, Mount Washington, KY 40047 Cost: ALL FREE Event Description: Join us for our first Christmas Gift GIveAway here at The River Mount Washington. During our GiveAway we will have a time of Christmas Carols, Candy Canes for everyone, and a great Christmas Story shared! All of these moments will take place inside of our Church Sanctuary. Concluding the caroling and christmas story shared, Santa Claus and his elves will begin to pass out toys to each Boy and Girl present. Pictures with Santa are welcome if you desire. (IMPORTANT: FREE Registration is required for this event. When you register your child/children, this helps us to ensure we have a toy for Santa to give them.) Following the distribution of Gifts, Various prizes will begin to be given out to each winning raffle ticket. Raffle Tickets are given when you CHECK IN at our Registration Station for each registered attendee. A Valid Photo - ID for adults is required when checking in. NO Photo - ID is required for the kids. Raffle ticket winners will have the chance to win: Gift Cards, Various Prizes, and a Bill Pay. (Bill Pay is a desginated amount towards an indivuals bills) When all prizes are given out, our team will direct all attendees to our Fellowship Hall to pick up a Grocery box, and any clothes needed. This Event is scheduled to begin at 11am for attenedes to begin checking in, and the festivities will start no later than 11:30am. PLEASE NOTE: As this is a completely FREE event, REGISTRATION IS REQUIRED to ensure each Family rececives one toy each for each child, and food box. *When Registering Yourself and all party members, you are agreeing to the Release and Wavier of Liability form below.* Release and Waiver of Liability Form I ___________________________________________, HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THE RIVER MOUNT WASHINGTON/CHURCH OF GOD OF PROPHECY, INC’s (hereinafter THE RIVER) Christmas Gift GiveAWay, including but not limited to any risk that might arise from negligence or carelessness on the part of the persons or entities being released, because of their possible liability without fault, or from danger to any real or perceived exposure to COVID-19 virus through participation in this event and or future events held by The River Mount Washington and not limited to events held inside building of The River Mount Washington as well. I certify that I am physically well, I do not have any symptoms of COVID-19 and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity, and or future events by The River Mount Washington. A. I WAIVE, RELEASE, AND DISCHARGE, COGOP/The River and/or their directors, officers, employees, volunteers, representatives, and agents from any and all liability, including but not limited to, liability arising from negligence or fault of the agents of COGOP/The River or other participants of this event, for any real or perceived exposure to COVID-19, Illness, death, disability, personal injury, property damage, or actions of any kind which hereafter occur to me including my traveling to and from this activity. B. INDEMNITY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this Waiver and Release From Liability form from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. I acknowledge that they are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting specific activity on their behalf and they are not responsible for any real or perceived exposure to COVID-19. I acknowledge that this activity and or future events may involve direct physical contact individuals other than my immediate family and/or household members. I acknowledge that I am attending this event and or future events of my own free will and COGOP/The River and/or their directors, officers, employees, volunteers, representatives, and agents did not force or compel me to attend. I acknowledge that I have been to public places, businesses, and/or been in the presence of individuals other than those in my household before and will be again after this COGOP/The River event. This Waiver and Release from Liability Form shall apply to all minors in my legal care, custody, and control attending this event. This Release from Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable in the State of Kentucky.
