Here's a look at where we are offering the vaccine to eligible individuals the week of December 6: Monday, December 6 at Annette Winn Elementary Tuesday, December 7 at Beulah Elementary Wednesday, December 8 at Dorsett Shoals Elementary Thursday, December 9 at Factory Shoals Middle Friday, December 10 at Factory Shoals Elementary from 2:00 pm - 7:00 pm at each location. For children's vaccinations, proof of parental consent is required. Please be prepared to provide the last 4 digits of the child's social security number or a copy of parents insurance. If coming for 2nd, 3rd or booster doses, please bring your current CDC vaccine card. This vaccine event will be open to the public.
