CalHIVE's first network-wide one-day learning workshop focused on fundamentals of population health management for provider organizations. This highly interactive one day workshop will provide an overview of population health management for provider organizations with a focus on the practical application of these concepts. Participants:  The event is offered to CalHIVE teams as well as extended members of participating organizations. Organizations should bring at least three individuals and no more than five.  The course is designed to support CalHIVE teams and individuals who directly lead or manage population health functions, including patient outreach, patient engagement, risk stratification and reporting.  Agenda: The day will start at 8:00 AM for breakfast and networking and will close by 4:30 PM. A detailed agenda will be released in early January 2022. The day will include expert presentations, peer networking and team time. The workshop will cover the following topics: Patient-Centered Engagement & Care Needs Screening Understanding Patient Profile: Risk StratificationApproaches to Complex Patients Person-Centered Care PlanningImpactful Outreach for Patient ActionCare Coordination  Cost:  There is no cost to attend this training for CalHIVE participating organizations. Travel Reimbursement: CQC will reimburse travel for up to five attendees per organization. Details will be shared in early January. Cancellation Policy:  If you have registered but are no longer able to attend, please let us know 7 calendar days in advance of the event. Faculty:  Experienced CQC staff bring a ‘real world’ perspective to this training by offering lessons learned and challenges from prior quality improvement work in various healthcare delivery systems. Faculty take a learner-centered approach to the delivery of course concepts and tailor each training to participants’ needs.
