Dog Scratching and Chewing Fur Off

I put a new tick & flea collar on her 2 months ago and the problem still persists. She is chewing rear legs. She also has chewed fur off around her anus and her underside/crotch area. Also scratching her front leg, "shoulder" area on both sides. I tried a spray on from Petco but it's very hard for me to hold her to spray her affected areas. Can anyone make any suggestions? I hate like hell to have put a lampshade on her to stop the chewing It seems like it would almost be torture to her if she's still itching and not able to scratch or chew.
