All supported Living Positions require the following minimum staff member qualifications, skills or education, to ensure that our Direct Care Personnel provide the highest quality support possible to our consumers. 1. The skill, training, or education to do the following: (a) Establish and maintain constructive and appropriate relationship with consumers. (b) Minimize risk of endangerment to the health, safety, and well-being of consumers. (c) Certified in first aid and CPR 2. Responsible for implementation of the Consumer’s individual support plan. (a) Direct care Staff/Support coach shall participate in; (b) The development of client’s support plan and by following individualized service plan/schedule (c) Train consumers to acquire their Individual Program Plan (IPP)/ Individual support Plan (ISP) Objectives, assisting consumers with their personal needs/care and involving them in outside community activities. (d) Train consumers to follow all medication administration procedure and maintain appropriate documentation. (e) Document notes on each training objectives (f) Follow all safety regulations (g) Accompany consumers for their medical appointments 3. Have a current and valid license and certificates, which are legally required to provide services Each employee must have the following items completed; • FBI finger print clearance • Current Negative TB Test • Current CPR/First Aid card • Current Driver’s License • Current Vehicle insurance • Current DMV printout • Three Reference letters • At least Six months experience working with Adult with Developmental Disabilities. 4.
