Open to all Illinois residents ages 5* and older, regardless of insurance coverage and/or immigration status. *Please bring proof of age eligibility for kids, e.g. birth certificates, passport, or school records WHAT: This limited event requires you to RSVP to get a FREE timed-entry ticket to help us manage the line, and ensure we meet the demand for supply of Pfizer doses and Moderna & Pfizer boosters for adults WHERE: LaSalle II Magnet School- 1148 N. Honore St PLEASE READ OVER THE FOLLOWING EVENT DETAILS CAREFULLY: Please sign up for a timed entry ticket to help us manage community expectations, access to the event, and vax supply. 1. TENTATIVE PENDING FEDERAL APPROVAL: This event will only take place upon approval of the Pfizer COVID-19 Vaccine for children ages 5-11 (expected prior to event date.) 2. ACCESSIBLE TO ALL: This event will be open to ALL Illinois residents ages 5+ for doses 1, 2, and 3 (boosters). No insurance needed. No proof of immigration status required. No questions asked. 3. COVID-SAFE: Masks for all are mandatory. If you are sick, stay home. 4. WHAT TO BRING: Proof of age eligibility (e.g. birth certificates, passport, or school records)Vaccine cards for anyone receiving Dose 2 or boostersInsurance, if you have it (there is no co-pay for insured patients)Required TIMED ENTRY TICKET from Eventbrite5. PLANNING Parking is street and neighborhood only, subject to restrictions. We recommend public transportation and ride shares. Dress in loose clothing to provide vaccine access to upper arm. Please arrive during the hour-long window of your TIMED ENTRY TICKET. Plan to wait in line, masked, distanced, for registration, vaccination, and then for 15-30mins after vaccination for the observation period for anyone receiving first dose. Registration, Vaccination, and Observation all take place on site.
