I need someone to replace a 5 bolt tire on a 2007 Honda Odyssey

Here's what's going on. The rear passenger side tire is losing 10 PSI a week. I had someone look at it and there is no visible damage. HOWEVER, the tire has been patched. So I'm thinking there might be a hole in the patch itself. So what I would need is either for you to find the needle in the haystack and patch the patch or for you just to replace the tire itself. I also don't have the tools. So you would need to supply things like a PSI meter, a tire jack, and the wrenches.
