The Township also asks that those planning on attending any of the clinics wear a short sleeve shirt and arrive in good health. APPOINTMENT REQUIRED: REGISTER HERE Friday, November 19th 11:00 AM to 1:00 PM At Orland Township 14807 South Ravinia Ave. Orland Park, IL 60462 BOOSTER REQUIREMENTS Must be at least 6 months from 2nd dose in order to receive 3rd dose. (2nd dose date of April 28) Bring completed/signed consent form Form can be completed online and print off, or completed on site. Bring copy of driver's license Bring copy of insurance card Bring CDC vaccination record card that shows previous 2 shots BOTH MODERNA & PFIZER AVALIABLE
