JR 2298 Case Manager III Santa Barbara, CA 93103 Salary: Starting at $21.00/hr DOE Full Time Non-Exempt PATH (People Assisting The Homeless) is seeking candidates with case management experience to join our Santa Barbara PSH Program as a Case Manager. WHAT WE DO Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. PATH Santa Barbara is open 24 hours a day, 365 days a year and is dedicated to assisting homeless individuals achieve greater health, wellness, and self-sufficiency, by helping as many as possible access the services they need to improve their health, find employment and transition to stable housing. Our new scattered-site Permanent Supportive Housing (PSH) program provides permanent housing and on-site services to individuals who are currently experienced homelessness. WHAT IS A CASE MANAGER? The Case Manager works in the field as part of the PSH team to provide case management services designed to assist in obtaining and maintain stable housing to individuals referred to the program by the County of Santa Barbara. Responsibilities of the Case Manager include: • Conducting intake and needs assessments for participants • Collaborating with participants to develop individualized housing and service plans • Meeting with participants on a weekly or more frequent basis to provide ongoing support to ensure service plan progress • Securing appropriate levels of supportive services through referrals, linkages and advocacy for identified needs including mental and primary health care, substance use treatment, job counseling and in-home supportive services • Working with participants to establish a plan for lease-signing and move in • Collaborating with the PATH Lease Up program and other agencies to find and secure units for participants • Utilizing and developing community resources to broker and link participants to services WHAT YOU BRING We’re looking for candidates that: • Maintain and execute confidential information according to HIPAA standards • Demonstrate knowledge of and experience with evidence-based case management techniques including harm reduction strategies, crisis intervention techniques and motivational interviewing • Are proficient with technology and computers, particularly Microsoft Office Suite • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs • Work just as well independently as they do on a team • Exercise mature judgement, and are highly motivated, self-starting and proactive • Are excellent at communicating, whether in writing or verbally • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment MANDATORY REQUIREMENTS For this role the Case Manager must: • Hold a Bachelor’s Degree in a related field • Have at least 1 year of case management experience with highly vulnerable populations, particularly those with health, mental health, and substance abuse issues • Be able and willing to work flexible hours which may include evenings or weekends • Have employment eligibility verification • Have or be able and willing to obtain CPR/First Aid training • Provide proof of full COVID-19 vaccination • Have reliable transportation and: • A valid driver’s license • Auto Insurance • The ability to qualify for PATH insurance coverage • Successfully complete the following as a condition of hire: • Tuberculosis Test • Background Screening • Drug Test COVID-19 VACCINATION Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19.
