Description
The MPF (Mullen-Polk Foundation) is seeking families or individuals who would like to become a resource for youth. The Special Receiving Care Homes would become licensed to care for youth between the ages of 5 through 18.
After licensing has been completed, the family/individual would take placement of youth that have been placed into foster care, the family/individual would work with the MPF's case manager to complete assessments and get the youth set-up with the services that he needs, but not limited to doctor, dental, vision and mental health appointments.
The MPF focuses on the youth strengths, challenges/needs, behaviors, interpersonal relationships, medical and health needs, developmental concerns, education plan, safety (concerns/issues/plan).
This information is collected within 60 days of the youth placement to ensure that we can find the right long-term placement for the youth in your care.
The Special Receiving Care Home would have placements for a duration of 60 days maximum, and compensation would be $2,280.00 per month, per child.
