Pfizer COVID-19 vaccine for kids ages 5-11, this FREE vaccine clinic is open to all ages 5+. No insurance needed. Open to all Illinois residents ages 5* and older, regardless of insurance coverage and/or immigration status. *Please bring proof of age eligibility for kids, e.g. birth certificates, passport, or school records WHAT: This limited event requires you to RSVP to get a FREE timed-entry ticket to help us manage the line, and ensure we meet the demand for supply of Pfizer doses . Kemper Place , 60614) WHEN: Friday, November 12 (Dose 1) 9AM-5PM in the Lincoln Cafeteria (Door 14) Saturday, December 4 (Dose 2) 9AM-5PM in the Lincoln Cafeteria (Door 14) PLEASE READ OVER THE FOLLOWING EVENT DETAILS CAREFULLY:Please sign up for a timed entry ticket to help us manage community expectations, access to the event, and vaccine supply. ACCESSIBLE TO ALL: This event will be open to ALL Illinois residents ages 5-11 for dose1. No insurance needed. No proof of immigration status required. No questions asked. COVID-SAFE: Masks for all are mandatory. If you are sick, stay home. MAKE SURE TO PRE-REGISTER: Instavaxx registration link will be shared soon. WHAT TO BRING: 1. Print out of pre-registration forms 2. Photocopy of age eligibility (e.g. birth certificates, passport, or school records) 3. Photocopy of insurance, if you have it (there is no co-pay) 4. Required TIMED ENTRY TICKET from Eventbrite *November 12th date is dependent on vaccine supply. The U.S. government has placed orders for every child and supply shortages are not expected. WHAT TO BRING: 1. Print out of pre-registration forms 2. Photocopy of age eligibility (e.g. birth certificates, passport, or school records) 3. Photocopy of insurance, if you have it (there is no co-pay) 4. Required TIMED ENTRY TICKET from Eventbrite PLANNING Parking is street and neighborhood only, subject to restrictions. We recommend public transportation and ride shares. Dress in loose clothing to provide vaccine access to upper arm. Please arrive 10 minutes prior to your ENTRY TICKET time. Plan to wait in line, masked, distanced, for registration, vaccination, and then for 15-30 mins after vaccination for the observation period for anyone receiving first dose. There are no confirmed appointments for this event. The TIMED ENTRY TICKETS help us manage expectations for attendees, and help us plan for adequate vaccine supply. Registration, Vaccination, and Observation all take place on site. There will be no photo or video allowed on site to protect patient privacy. This is a FREE and volunteer-run event. Questions?
