I live in Northern, Michigan and need a substance abuse evaluation/assessment done online. It is for a driver's license hearing. It needs to be done by a mental health professional, counselor/therapist, or medical health professional. I would fill my part of the form out then send it to you. All you would have to do is fill in 2 small sections and sign it. I would pay $50 (negotiable).
