Chimney cap repair

Hi, I need my chimney cap to be repaired and painted with metal paint. It is a little inclined, so if you can fix, make it straight, attach it firmly or seal it and then paint it. The height of the chimney is approximately 28 feet, but it can be reached from the roof. The height above the roof is approximately 5 feet. The chimney is at the end of the house. This is two story townhouse. Thanks
