Serving kids ages 4 - 15 years old. Every kid must be registered and have ticket upon arrival in order to receive toy. Photo Policy: I agree to grant 88 Blessings, Inc. and its authorized representatives’ permission to record and photography film and/or video, pictures of me and my family during all 88 Blessings, Inc. I further agree that any and all materials photographed or videoed may be used in any form, as part of any future publications, brochure, or other printed materials used to promote 88 Blessings, Inc. and further such use shall be without payment of fees, royalties, special credit or other compensation. I understand and confirm that being a recipient of 88 Blessings, Inc. toy drive. I hereby release 88 Blessings, Inc., a nonprofit corporation Allen Hurns and all of its affiliates and representatives from ANY and ALL liability, claims, and demands during an 88 Blessings, Inc. event. I am in good health and suffers from no physical or mental condition that would create an unreasonable risk of harm to himself or herself, or to any other participant. I hereby assume all risks, known and unknown, foreseeable and unforeseeable, in any way connected with my participation. I accept full responsibility for any liability, injury, loss, damage or death in any way connected with my participation. This paragraph shall be inclusive of, but not limited to, any injury or death resulting from exposure to food or beverage allergies, food borne illnesses, theft of or damage to my property, transportation or commute, exposure to adverse weather conditions, or choking. On behalf of myself, hereby forever and unconditionally releases 88 Blessings, Inc. S.O.A.R Movement its owners, officers, employees, agents, successors in any interest and arising out of, or resulting from, participating in this event. I further acknowledge that 88 Blessings, Inc. IS NO WAY LIABLE for any injury to resulting from my behavior. By submitting this application, I affirm that the facts outlined in it are true and complete. I understand that if accepted as a recipient, any false statements, omissions, or other misrepresentations made by me on this application may result in my kid immediate dismissal.
