need help with patching some carpet holes

When remodeling our bathroom, we relocated a doorway which createsd 2 approx 4”x4” holes. I have a 12x12 scrap of the same carpet and the matching pad. Also may possibly need some tack strip and stretching at the new threshold to the bathroom.
