Description
FREEHOLD, NJ — CentraState Medical Center is hosting a COVID-19 walk-in vaccine clinic with first and second doses, as well as booster shots.
Doses will be administered at the Vaccine Center (entrance 6) of the CentraState Medical Center, located at Star and Barry Tobias Ambulatory Campus, at 901 West Main Street.
No registration is required but make sure to bring your health insurance card and fill out the forms available here before arriving.
Those who are receiving a second dose or a booster dose should also bring their CDC vaccine card.
Here are the dates and times of the clinic:
Sept. 30, 3 to 7 p.m.Oct. 5, 8 a.m. to noon (ages 65+ only)Oct. 5, 1 to 5 p.m. (ages 12+)Oct. 11, 8 a.m. to noon and 1 to 5 p.m. Eligibility for the booster shot will be based on the following CDC guidelines:
People 65 years and older and residents in long-term care settings should receive a booster shot of Pfizer-BioNTech's COVID-19 vaccine at least 6 months after their Pfizer-BioNTech primary series.People aged 50–64 years with underlying medical conditions should receive a booster shot of Pfizer-BioNTech's COVID-19 vaccine at least 6 months after their Pfizer-BioNTech primary series.People aged 18–49 years with underlying medical conditions may receive a booster shot of Pfizer-BioNTech's COVID-19 vaccine at least 6 months after their Pfizer-BioNTech primary series, based on their individual benefits and risks.People aged 18-64 years who are at increased risk for COVID-19 exposure and transmission because of occupational or institutional setting may receive a booster shot of Pfizer-BioNTech's COVID-19 vaccine at least 6 months after their Pfizer-BioNTech primary series, based on their individual benefits and risks.
