FUSD WellPATH Flu Shot Clinic and Wellness Screening Onsite Event at Department of Prevention and Intervention The event is free and available for employees with district medical insurance, covered spouses, domestic partners dependents age 18+, early retirees, and Medicare-eligible retirees. ALL PARTICIPANTS REGISTERING MUST BE AT LEAST 18 YEARS OLD. Eligible participants can choose to receive a flu shot only, wellness screening only or both at this event. Must present district issued medical ID card. By Appointment Only! Please make sure to select the appropriate ticket type related to the service you would like to receive. If you would like a flu shot and wellness screening, you will need to have a ticket time for each event in your cart before proceeding to check out. Members that complete the wellness screening, will be given a $25 Visa gift card at the event. 5 Important Things to Know Before You Go 1. Pre-registration is REQUIRED 2. Masks must be worn correctly for the duration of the event 3. Social distancing practices/signage must be followed 4. Please do not arrive at the event until 5 minutes before your scheduled appointment time 5.
