HONOLULU — An Oak Lawn woman accused of using a fake, misspelled COVID-19 vaccination card when visiting Hawaii last month, failed to appear for a scheduled court hearing Wednesday morning in Honolulu when she had the option of showing up via Zoom, according to a warrant for her arrest. Chloe Mrozak, 24, was to appear before a judge in a Honolulu courtroom for a status hearing, after she was charged with two misdemeanors on Aug. 31 for falsifying documents. When she failed to show up in her court, a $500 bench warrant was issued for her arrest. >>> Oak Lawn Woman Accused Of Using Fake 'Maderna' Card In Hawaii The Oak Lawn woman made headlines when she allegedly presented a fake vaccine card with the name "Moderna" spelled "Maderna," to bypass Hawaii's mandatory 10-day quarantine period for out-of-state visitors on Aug. 23. She was arrested a week later when she showed up for her departure flight at Daniel K. Inouye International Airport in Honolulu after vacationing for five days in Hawaii, according to court documents. Mrozak is also accused of falsifying information on her Safe Travels Hawaii itinerary, required for visitors to the state, stating she would be staying at the Holiday Inn Express in Waikiki, which had no record of her reservation. Following her arrest, Mrozak was detained for five days in the Oahu Community Correctional Center. Mrozak appeared before Judge Karin Holma for a bond hearing on Sept. 1. In a video of that hearing on Hawaii News Now, Mrozak made a tearful promise that she would follow through on all her scheduled court appearances. "Ms. Mrozak, if this court releases you, are you going to make all your court appearances," the judge asked. "And you are allowed to appear by Zoom." "I promise, I promise, I will do whatever it takes," Mrozak said in the court video. "I swear I'm not a bad person." Mrozak was released, despite the objections of the Hawaii prosecutor. A copy of the warrant provided to Patch by Hawaii News Now authorizes the chief of police, deputy sheriff or any officer authorized to make arrests in the State of Hawaii to take Mrozak into custody. "Defendant, CHLOE MROZAK, failed to appear at the hour of 08:30 AM on September22, 2021 before the above-entitled Court, at Honolulu, Hawaii, and/or knowinglydisobeyed or resisted the process, injunction, or other mandate of the above-entitledCourt, in the above-entitled matter," the warrant states. In addition to the misdemeanor charges, Mrozak is now being charged with contempt of court. A bail of $500 has been issued for her arrest. It is unknown if Mrozak left Hawaii. A residence at her last known address in Oak Lawn was dark and no one appeared to be at home. The arrest warrant is not transferable outside of Hawaii. However, should Mrozak surface on the islands or return to Hawaii in the future, she will most likely be arrested. Since Mrozak made headlines for allegedly falsifying her COVID-19 vaccine card and misspelling Moderna with an "a," two other people from Georgia have also been charged with falsifying COVID-19 vaccine cards.
