INSURANCE REQUIREMENTS ARE AS FOLLOWS: A. Must provide a certificate from your company’s insurance agent, naming the Company that you were subcontracted by as an additional insured and certificate holder. B. Must provide proof that the insurance coverage is both primary and non-contributory. C. Coverage must also include completed operations coverage. D. Proof of the following policies must be provided: • Workers Compensation • General Liability – minimum general aggregate is $2 million / $1 million per occurrence • Umbrella – can be forsaken if the General Liability policy carries a minimum general aggregate of $4 million / $2 million per occurrence. • Auto
