Bring Insurance card(s). Both Medicare and supplemental insurance cards Stay at home and/or cancel your appointment if you are sick. Bring and wear masks throughout the clinic (even if not receiving a shot). Bring your own snacks for recovery purposes. Wear short sleeves. Arrive at scheduled time.
