NTTAP Learning Collaborative Spotlight: Team-Based Care The Team-Based Care Learning Collaborative is a free eight-month participatory experience designed to provide knowledge, tools, and coaching support to help health centers and Look-alikes develop highly trained clinical primary care teams that will enhance their model of high-performing team-based care. This will include: A series of videoconference learning sessions and coaching sessions with 10-12 primary care teams from across the countryQuality improvement trainingOngoing mentoring for coaches in your organizationTechnical assistance; andAccess to web-based tools. The Collaborative is designed for teams beginning or restarting their move to high performance team-based comprehensive primary care with knowledge about the basic principles and best practices of care and the strategies to plan for implementation. Each participating FQHC and Look-Alike will: Identify a clinical teamIdentify organization leadership representationIdentify a designated coach to lead the team through the work of the Collaborative The Team-Based Care Collaborative will engage teams in work focused on implementing and measuring outcomes for a model of team-based care best suited to their organization through: Using assessments of their current team-based care model to identify areas for process improvement and role optimization.Developing a problem statement based on dataUsing quality improvement concepts and skills with coaching support to systematically achieve one or more specific aims.Developing a specific aim, global aim statement, and process map based on the problem statementDeveloping and utilizing a Team-Based Care playbook in your organization to support spread Apply Now More information can be found in the application at this link Community Health Center, Inc.
