Press release from the Town of Concord: September 8, 2021 The Concord Council on Aging (COA) is seeking volunteers who can assist older adults obtain the information they need to manage their health, improve their quality of life, and live independently in the community. The COA is starting a "Patient Navigator" program where new volunteers will be required to complete five hours of comprehensive training before being assigned to work with a senior member of the community.  Valerie Boggia, Geriatric Health Nurse at the COA, will coordinate the program.    A COA Patient Navigator volunteer will: Gain experience assisting older adults in navigating their healthcare systems  Join a team of highly trained volunteers who make a direct and positive impact in the lives of older adults  Attend five hours of training and be willing to make a minimum 6-month time commitment to the program; provide a copy of their auto insurance Learn How to: Help older adults prepare for upcoming medical appointments, review concerns and frame questions Drive to and serve as a companion at medical appointments  Facilitate communication between doctor and patient Take notes during the visit Assist in scheduling follow up appointments and clarifying recommendations and medication changes The Older Adults Helped Are: Residents of Concord aged 60 and older Able to enter and exit vehicles independently   If you or anyone you know is interested in this worthy volunteer opportunity, please contact the COA: Valerie Boggia, RN (Tuesday – Thursday) 978-318-3016 or [email protected].  You may also call the COA during weekday business hours at 978-318-3020.
