Concrete Pour/Slab

I need a concrete professional to come pour a slab in the bottom floor of my building. I've just ripped out the old subfloor and joists. Need about 2 inches of gravel, then eps/xps foam insulation and then about four inches of concrete to get to grade. If you can also do a polished finish floor that's a plus, but not required. Looking to start ASAP.
