Create 4 small forms and pour concrete in my backyard

Hello, Are you experienced with pouring concrete? I need someone experienced with concrete to build 4 forms and pour concrete. The forms need to be 12 inch x 12 inch x 6 inch. You will have to dig out some dirt to make a small shallow hole of this size to build the forms in the dirt area in my backyard. The top of the concrete needs to be level, smooth and line up with the dirt in the backyard. You will have to go to the nearest Home Depot (4 miles away) to pick up the right amount of quikcrete (quick setting concrete) and to get some wood for the forms.
