What Is CDPAP? The Consumer Directed Personal Assistance Program (CDPAP), Medicaid eligible individuals loving in the State of New York can choose a family member* or friend to be their paid caregiver. Now, they can receive care from someone who takes their health and well-being personally. To register with Health Aide inc., Medicaid Consumer must agree to enroll into a participating Managed Medicaid PACE  (Program of All-Inclusive Care for the Elderly) Program. Health Aide inc. CDPAP offers the following Caregiver pay rates: NYC (Brooklyn, Queens, Staten Island, Bronx, Manhattan): $21.50/hour plus Paid Sick Leave Nassau, Suffolk, and Westchester counties: $21.50/hour plus Paid Sick Leave CDPAP requirement WITH PACE Program Currently Medicaid eligible and/or entitled to Medicare Part A and/or enrolled in Medicare Part B or able to pay privatelyLiving in our service area including the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Suffolk and Westchester countiesIn need of nursing home level of care, but able to remain living safely in your home. You must need more than 120 days of community based long-term care services.Being 55 years or older Caregiver requirements: Valid ID(s)Physical with Lab WorkTB test (2 PPD or Quantiferon)Criminal History Record Check (CHRC)Fingerprinted and processed by the NYS Division of Criminal Justice Services (DCJS)*Spouses and designated representatives are not eligible to be personal assistants. www.healthaide.org347.620.6226
